Seems like any little change in stays can make a big change in fit (or no change at all, there's no rhyme nor reason to it). If it's a large bust you're trying to fit with narrow shoulders, would you just widen in the front chest and under the arm, but keep the back narrow? (I am just pondering. I have no experience with fitting such a figure.) For sleeve range of motion, I am a big fan of just making the curve of the sleevehead shallower. Lower it at the top, raise it at the bottom, and, if it is too small for the armscye, slash and spread the sleevehead so it is longer horizontally. Good luck!
